We are pleased to inform you that the World Heart Federation (WHF) is currently working on developing a new Roadmap for Cardiovascular Disease and Comorbidites and Integrated Care. As part of this initiative, we are seeking your valuable input to help shape the content of the Roadmap and WHF’s future activities related to CVD and comorbidities and integrated care.
What is the survey about?
This brief survey includes a series of questions focusing on:
- Background information on your work setting.
- Current realities, needs, and barriers regarding people living with CVD and comorbidities (diabetes, chronic kidney disease etc)
- Existing and opportunities to implement integrated care for CVD and comorbidities.
The survey will take approximately 10-15 minutes to complete.
Who should be involved?
We invite healthcare professionals involved in treating patients living with CVD and comorbidities (diabetes, chronic kidney disease etc) to share their perspectives. Our aim is to collate the views of as many countries as possible. We therefore ask each of our Members to share this survey with relevant individuals who they feel can appropriately take part in providing this feedback. This is your opportunity to offer the feedback and input of your country or region, so we strongly encourage your participation.

The feedback provided through this survey will directly contribute to the development of a comprehensive roadmap to address challenges related to CVD and comorbidities and integrated care.
